Assisted Living Program - Aging at Home Initiative
The "Aging at Home" Initiative aims at reducing the number of emergency visits, and at avoiding premature admissions to long-term care facilities or hospitals. Services are intended to allow seniors to stay healthy, and to live independently with dignity in their homes.
Subsidized Assisted Living Services under the "Aging at Home" Initiative include:
- Meals-On-Wheels
- Personal Support Services: bathing, vitals monitoring, medication supervision/reminders, complete interventions identified in professional care plan; social support.
- Housekeeping/ Laundry Assistance
- After Hours Emergency Response
- Eligibility, type and frequency of services are determined for each tenant based on results of personal assessments of self-maintenance, activities of daily living, and nutritional status. Services are offered Monday through Friday from
8:30 am to4:30 pm .
